Forehead Reduction

What is Forehead Reduction?

Forehead reduction is commonly known as hairline lowering. It is a surgical procedure that assists in reducing the forehead size by moving the hairline forward. The advantage of using this technique is that it can be combined with hair transplant to give a denser and lower hairline. Also, it is one of the frequently performed procedures in Facial Feminization surgery.

Forehead Reduction in Male vs Female Patients

Men and women with high foreheads frequently wear hats or bangs to help balance their faces, but these are only temporary fixes. Although the aesthetics and surgical techniques for male and female patients are quite similar, still there are certain differences in hairline designing and planning of the procedure.

The hairline of a woman is usually five to six cm above the brows. Women with high hairlines or broad foreheads may struggle with self-confidence and could have problems selecting a suitable haircut that covers their forehead. Because men's foreheads are normally longer than women's, women frequently see a high forehead as a masculine feature.

A man's forehead is usually six to eight cm above his brows. An extremely high forehead might make a guy feel self-conscious about his looks since it resembles a receding hairline. Men who have not undergone hair transplant procedure and do not have male pattern baldness may benefit from forehead reduction. Male patients should be aware that, because of the gradual nature of hair loss, they may need further treatments in the future to preserve their surgical forehead reduction outcomes.

Ideal Candidates for Forehead Reduction

A forehead reduction operation can help women with a high hairline. For men and women who are unhappy with the size of their foreheads and general look, Dr. Shilpi Bhadani recommends forehead reduction. Individuals with a high forehead, thick hair, and pliable forehead skin who have no personal or family history of hair loss may be suitable candidates for this surgical surgery.

However, men are not always the ideal candidates for a forehead reduction. Men with thin hair or age-related male pattern baldness benefit more from our Hair-ART transplant surgery rather than a forehead reduction, as per Dr. Shilpi Bhadani (Mch Plastic Surgery).

During the consultation, Dr. Shilpi Bhadani discusses openly with each patient about their concerns and goals and gathers information about prior surgeries along with medical history to determine the individual needs and decides upon the best treatment option for the patient. She also recommends alternative options for individuals with a personal or family history of progressive hair loss, as future hair loss could reveal the incision. Patients with a history of abnormal scarring or issues with wound healing may not qualify as candidates for forehead reduction.

Requirements for the Surgery

This surgery necessitates scalp flexibility or stretchiness, and tissue expansion may be performed to have the desired outcomes. Dr. Shilpi Bhadani also recommends hair-ART Hair transplant surgery to hide scar tissue and provide a more natural-looking hairline in some of the cases.

Hairline Lowering Procedure

At SB Trichology (a unit of SB Aesthetics which is state-of-the-art Cosmetic, Plastic and Maxillofacial Surgery facility) our surgeons believe that hairline lowering surgery is more of art than a surgery to create a perfectly balanced hairline. The most preferred choice to perform this surgery at our centre is under general anaesthesia. Dr. Shilpi Bhadani places a single incision on the scalp i.e. called as trichophytic or trans-follicular incision within the hairline thus removing excess skin from the forehead and scalp.

The doctor cleverly moves the scalp forward to form a new and lower hairline.
Dr. Shilpi also performs the surgery to contour the bony orbital structure of the forehead giving a pleasing shape during the procedure, using the same incision if required and this is well explained to the patient before surgery. The sutures are placed in an angulated manner so that it allows hair growth in front and within the scar for natural-looking results. The duration of the surgery varies from 2-4 hours depending on the extent of changes to be made.

Two-Stage Hairline Lowering

Certain individuals who have high hairline and tight scalp are the ideal candidates for two-stage procedure. During the first stage, we insert balloon-like expanders also called tissue expander underneath the skin of the scalp. The expanders are inflated progressively and stretch the scalp over six to eight weeks. The second step includes the surgery during which we remove expanders and lower the hairline.

Hair Grafts with Forehead Reduction

We may advise Hair-ART FUE Transplant to the lowered hairline. This includes removal of follicular units from the back of the head, separating them based on the type of the follicle and then placing them in the slits created on the lowered hairline.

Hair-ART FUE Transplant

There are candidates who may not be benefited from the surgery and in these patients Dr. Shilpi Bhadani advocates the Hair-ART FUE Transplant to lower the hairline of candidates. The specific reason for advising these patients to undergo Hair-ART FUE Transplant and not a hairline reduction surgery is insufficient scalp laxity. Typically, 2 sessions performed at a gap of 6-8 months help to achieve the desired hair density. The Hair-ART FUE Transplant will create a lower hairline and can provide results that are like those produced with hairline lowering surgery.

Hairline Lowering Recovery

Our team provides each patient with detailed post-surgical instructions and are advised to be followed closely to encourage proper healing and understand the course of events that the patient will encounter post-surgery.

The healing process varies among patients, the recovery typically takes two to three weeks, and the results keep on improving with every passing day upto 6 months.

Mild swelling and discomfort are expected with numbness initially for the first few days after surgery. Prescription medication generally includes antibiotic coverage and mild painkillers. We, at our centre, advise patients to have their first head wash on the 5th postoperative day.

Patients are advised to abstain themselves from heavy lifting and strenuous activities for 6-8 weeks. Most patients return to their basic daily regime after their stitches are removed, which typically occurs one week after surgery.
